Outreach Worker - East London

Job Overview:

Assist street outreach workers in providing support to rough sleepers. Engage with clients, assess their needs, and help them access accommodation and services. The role will have a mixture of early morning shifts and evening shifts. Street counts will be also a part of the role once a week.

Based in East London

Key Responsibilities:

• Conduct outreach Monday-Friday
• Collaborate with agencies to provide housing and support options
• Advocate for clients' welfare and healthcare needs
• Manage a caseload, offering guidance until clients are housed
• Maintain accurate case notes on CHAIN and other databases
• Manage a case load of up to 10 clients
• Ensuring clients are being accessed and moved over to the appropriate services according to their needs

• Pay: £13 - £17 per hour
